New Jurats Form

As of 2014 all certificate of Jurat must include the new disclaimer notice on the top of the certificate and enclosed in a box.  If your document requires a Jurat to be filled out by a notary, it must look like the following as pictured below with all of the proper components.  If it is not then we will use our form with all of the proper wording.
